TriStar Hendersonville Medical Center seeks a dedicated Physical Therapist to provide patient-centered care across the continuum of therapy services. You will perform assessments, implement care plans, and collaborate with physicians and care teams to optimize outcomes for diverse patients.

Responsibilities include home-based interventions when appropriate, patient and caregiver education, and efficient caseload management to support a high-quality rehabilitation program within a busy hospital

Qualificações

• Licensed Physical Therapist per state licensure.
• BCLS certification required within 30 days of start date.
• Bachelor's degree in Physical Therapy required.

Responsabilidades

• Provide evaluations and treatment within scope of practice.
• Assess functional status and develop the plan of care.
• Deliver therapy at the patient’s home when appropriate.
• Instruct patient/family in exercises and prosthetic use outside sessions.
• Prioritize caseload and coordinate staffing as needed.

Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.

What you will do in this role:

• You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• You will provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
• You will instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Licensed Physical Therapist
• Bachelors Degree

TriStar Hendersonville Medical Center is a 159-bed facility providing quality healthcare to Sumner and surrounding counties for more than 45 years.
