Job summary

Healthcare Outcomes Performance Co. (HOPCo) in Flagstaff, AZ, is seeking a Physical Therapist for an outpatient clinic. You will assess patients, establish goals and care plans with physicians and families, and contribute to interdisciplinary treatment planning.

Required are a current state license and at least two years of outpatient PT experience; a Doctorate in Physical Therapy (DPT) is preferred. The role includes supervising PT staff and maintaining confidentiality in a normal office

Qualifications

  • Current valid state physical therapy license.
  • Two years outpatient PT experience; orthopedics preferred.
  • Clinical Doctorate (DPT) preferred.

Responsibilities

  • Assess patient condition and develop goals and care plans with physician and patient/family.
  • Provide interdisciplinary treatment planning with physicians.
  • Document findings for the medical record.
  • Educate patients and families on therapies and exercises.
  • Supervise PT Assistants and PT Technicians.
  • Maintain patient confidentiality.

Job description

  • Assesses patient condition, establishes goals, treatments, and care plans in conjunction with the physician, patient/family.
  • Ensures that appropriate treatment is performed in an interdisciplinary fashion, consulting with physicians about the impact of physical therapy factors on the patient’s health status.
  • Documents findings, treatments, and other patient information for the medical record.
  • Educates patient/family on appropriate care and use of various therapies/exercises.
  • Serves as information/education resource for patients, families, and staff.
  • Supervises Physical Therapy Assistants and Physical Therapy Technicians.
  • Serves as a resource to Physical Therapy staff.
  • Provides ongoing education to the Physical Therapy staff.
  • Assists the director with patient care issues.
  • The job holder must demonstrate current competencies for the job position.

EDUCATION
  • Requires a Valid and active State Licensure
  • Clinical Doctorate (DPT) preferred.
Experience
  • Two years of experience in an out-patient physical therapy clinic. Experience with orthopedics is preferred.

Requirements
  • Current state license.
KNOWLEDGE
  • Knowledge of physical therapy principles, standards, and applications.
  • Knowledge of clinic policies, procedures, regulations, safety/infection control, quality control.
  • Knowledge of how to use physical therapy equipment.
  • Knowledge of medical terminology.
Skills
  • Skill in appropriate evaluation and treatment of patients.
  • Skill in effectively communicating with in-house as well as external physicians and referral sources.
  • Skill in using a computer system/EMR to accurately reproduce all appropriate documentation.
  • Skill in accurate recognition of patient/clinic danger and appropriate fall risk patients.
Abilities
  • Ability to establish/maintain effective working relationships with patients, providers, and staff.
  • Ability to supervise and mentor physical therapy staff.
  • Ability to communicate clearly in verbal and written form.
  • Ability to be a resource to patients and staff.
ENVIRONMENTAL WORKING CONDITIONS
  • Normal office environment.
  • Some travel within the community.
PHYSICAL/MENTAL DEMANDS
  • Requires sitting and standing associated with a normal office environment.
  • Some bending and stretching are required.
  • Manual dexterity using a calculator and computer keyboard.
  • Requires occasional support all size patients in transfers.
  • Ability to lift 50 lbs. Demands of kneeling, squatting, overhead lifting.
