Qualifications:

• Graduate of an accredited university with a B.S., Doctorate, MA, or MS in Physical Therapy recognized by the APTA.
• Current license and/or registration in the state (as applicable)

Job Responsibilities:

• Provide comprehensive physical therapy evaluations based on MD’s orders.
• Document findings accurately on standardized evaluation forms, following all guidelines.
• Develop and implement treatment plans with clear goals, modalities, and schedules.
• Obtain MD orders for evaluations, treatments, re-certifications, and discharges.
• Deliver treatment utilizing modalities and modify as needed, adhering to safety precautions.
• Accurately document patient participation and progress weekly.
• Update treatment plans and document discharges and in-services as required.
• Complete all necessary documentation, including care plans and MDS information.
• Participate in facility meetings and interdisciplinary team activities.
• Assist in scheduling, perform screens, and complete billing ethically and accurately.
• Cover for other therapists during absences and supervise junior staff or students as needed.
• Maintain professional relationships with staff, patients, and families.
• Provide in-service education and ensure compliance with policies, confidentiality, and federal rights.
• Report on time, adhere to hours, and project professionalism at all times.
• Meet productivity standards and follow HR policies regarding time off.
• Foster a positive work environment and perform additional duties as assigned.
