Job summary

Stonehill Communities is seeking a Physical Therapist to assess, treat, and plan therapy programs to enhance patients' physical abilities. You will supervise therapy staff, initiate referrals, and participate in care planning with the team.

The role includes screenings at regular intervals, establishing patient schedules with the team, and handling additional duties as assigned. This PRN position offers flexibility with 4-, 6-, or 8-hour shifts and a minimum of 16 hours per month.

Description

As a Physical Therapist, they provide services to patients, including assessment, treatment, program planning, implementation, and communication. They optimize the physical abilities and skills of patients by performing or supervising therapy staff in the following activities: initiating referrals, evaluating and treating appropriate patients, complying with minimum data set procedures, effectively participating in the care planning process. They conduct screenings of patients at regular intervals to determine the need for intervention/treatment. Establishes individual patient schedules in conjunction with other team members. Performs other duties or special projects as assigned.

Requirements

Minimum Qualifications:

• Bachelor's Degree in Physical Therapy from an accredited program.
• License to practice as a Physical Therapist within the State
• Ability to complete required in-service training requirements
• Must be able to move (walk, bend, sit, push, pull, lift) intermittently throughout the workday
