Job summary

Devinehealthandrehab is seeking a per diem Physical Therapist to join our in-house rehab team that serves residents with person-centered care. The PT will assess, plan, and administer physical therapy to promote healing and restore function in SNF/long-term care settings.

Requirements include an active Texas PT license and good standing. We welcome new grads and offer flexible scheduling and competitive compensation in a supportive, team-focused environment.

Qualifications

• Active Texas PT license in good standing.
• Experience in SNF/long-term care settings preferred.
• Familiarity with Net Health EDS is a plus.
• Strong interpersonal skills to manage diverse patients.

Responsibilities

• Consult with patients to assess their physical condition.
• Develop and implement individualized treatment plans.
• Administer physical therapy treatments to relieve pain and restore function.
• Document thoroughly and comply with regulatory requirements.
• Collaborate with the rehab team to ensure quality patient care.
• Attend required meetings with the Director of Rehab.

We are looking for aPTto join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
