Job summary

Progressive Rehab Solutions is seeking a licensed Physical Therapist to evaluate and treat patients in long-term care settings. You will develop individualized plans to improve mobility, strength, balance, and independence, then implement therapeutic interventions and monitor outcomes.

The role requires a DPT or equivalent, current state licensure, and collaborative teamwork with physicians and nursing staff. Travel to patient locations may be required.

Responsibilities

• Evaluate patients' condition, mobility, strength, balance; create individualized treatment plans.

• Provide gait training, transfer training, therapeutic exercises.
• Document evaluations, progress notes, discharge summaries per regulations.
• Collaborate with interdisciplinary team; supervise PTAs; communicate with physicians.
• Support continuing education and evidence-based practice.

Patient Care / InterventionImplement therapeutic interventions to improve mobility, strength, balance, coordination, and enduranceProvide gait training, transfer training, and therapeutic exercise programsUtilize manual therapy techniques and therapeutic modalities as appropriateEducate patients and caregivers on exercises, safety precautions, and home programsRecommend and train patients in the use of assistive devices (e.g., walkers, canes)C.
