Job summary

Big Spring Center for Skilled Care in Texas is seeking a Per Diem (PRN) Physical Therapist to join our in‑house team serving residents through person‑centered care and measurable outcomes. This role offers a $2,500 sign‑on bonus and flexible scheduling to accommodate your availability.

You will assess patients, develop and adapt treatment plans, document thoroughly, and collaborate with the interdisciplinary team to ensure high-quality care.

Qualifications

• Active Texas Physical Therapy license in good standing.
• SNF/Long term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with diverse personalities.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess and interpret patient evaluations and test results.
• Develop treatment plans using a variety of treatment techniques.
• Create personalized rehabilitation programs for patients.
• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment and adjust as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team to ensure quality patient care.
• Ensure documentation is thorough and timely and compliant with regulations.
• Attend required meetings designated by the Director of Rehab.

We are looking for a PT to join our in‑house team of outstanding professionals that service our residents through person‑centered care to achieve positive outcomes!

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
