Job summary

Amberwell Health is seeking a licensed physical therapist to deliver patient-centered care across inpatient, outpatient, and home health settings. You will evaluate patients, develop and execute personalized treatment plans, and document care plans, daily notes, and discharges in a timely manner.

Collaborate with a multidisciplinary team to optimize functional outcomes and support overall patient health within Amberwell locations.

Qualifications

• Post Graduate Degree from accredited physical therapy program.
• KSBHA license is required.
• BLS certification preferred.

Responsibilities

• Provide physical therapy services to patients in inpatient, outpatient, and home health settings.
• Develop personalized treatment plans after evaluating patient conditions.
• Complete evaluations, care plans, daily notes, and discharges in a timely manner.
• Collaborate with other health care professionals to optimize patient outcomes.
