Education
Bachelor's Degree and completion of a Physical Therapy program are required.
Experience
None
Licenses and Certifications
- PT - Physical Therapist license in Texas (upon hire)
- BCLS - Basic Cardiac Life Support (prior to providing independent patient care and maintained quarterly)
What You Will Do
- Perform age-appropriate patient assessments and establish care plans.
- Obtain and verify physician orders for therapy, ensuring appropriateness.
- Review medical records and obtain pertinent medical history from patients or families.
- Ensure patient informed consent for treatment.
- Evaluate patients using age and diagnosis-appropriate assessments and outcome tools.
- Integrate assessment results to identify functional problems and prioritize care with patient and family.
- Set measurable, functional goals with time frames, incorporating patient and family goals and preferences.
- Determine care plans and appropriate therapeutic interventions to meet goals.
- Initiate discharge planning in collaboration with the care team and patient.
- Demonstrate competence in age-appropriate assessments and treatments, individualizing care based on age-specific needs.
- Perform treatments within the scope of licensure, using appropriate techniques and ensuring patient safety.
- Evaluate patient response to treatments and reassess progress.
- Educate patients and families about treatment plans, safety, home programs, and assistive devices.
- Communicate with healthcare team members regarding patient care issues.
- Motivate patients to participate in therapy, including non-compliant or unmotivated patients.
- Utilize resources effectively to maximize efficiency and meet personal development goals.
- Participate in training and orientation of staff, students, and volunteers.
- Ensure care is provided responsibly, including supervision of licensed assistants and technicians.
- Attend departmental and community meetings and educational in-services.
- Meet productivity targets and represent the department positively.
- Complete work within scheduled hours and assist others as needed.
- Seek supervision or consultation when necessary and report issues to supervisors.
- Be willing to cross-train for treating various diagnoses and care levels.
- Participate in quality improvement activities and maintain a safe, compliant work environment.
- Collaborate with peers and other disciplines, participate in safety drills and emergency procedures.
- Document all assessments, care plans, treatments, education, and discharge summaries accurately and timely, following regulatory guidelines.
- Obtain physician signatures on plans and re-certifications, documenting all contact attempts.
- Ensure accurate billing and record procedures per departmental policy.
- Report unusual occurrences, injuries, and equipment malfunctions.
