Polaris Transitional and Extended Care Center - Anchorage, AK

Polaris Transitional and Extended Care Center in Anchorage, AK, is a skilled nursing facility offering long-term care, post-acute rehab, and geriatric outpatient therapy services. We provide a stable, engaging in-house therapy team and excellent programs for our geriatric clients' recovery journey. We are currently seeking a Physical Therapist for our rehab program.

Pay Range: $50.00-$65.00 per hour, based on experience

About our facilities:

The extended care facility is inspired by the Green House model, with 96 beds across 8 cottages, each with private rooms and communal spaces designed to foster social interaction. The transitional care facility is a 50-bed, high-tech environment with private rooms and a fully equipped therapy gym.

Anchorage, Alaska, offers a beautiful setting and outdoor adventures, with a diverse population including Alaska Native tribes. It’s an ideal location for an active lifestyle.

Duties:

1. Conduct patient screenings and evaluations, developing care plans adhering to standards.
2. Deliver rehabilitative treatments, including manual techniques, exercises, modalities, and wound care.
3. Provide consultation to patients, families, and caregivers regarding physical disorders.
4. Generate discharge summaries complying with regulations.

Qualifications:

• Valid Physical Therapy license required.
• Open to all experience levels, including new grads.
