FOX Rehabilitation in Plano, TX is seeking licensed Physical Therapists to deliver exceptional one-on-one care to older adults where they live, including private residences and senior living communities. You’ll help patients improve mobility, independence, and quality of life through the Geriatric House Calls model, with autonomy to schedule your own treatment plans.

This role offers flexible scheduling, potential telehealth options, and collaboration with an interdisciplinary team.

Qualifications

• Licensed PT in TX or eligible to obtain license.
• Graduate of an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote aging in place.
• Provide telehealth/virtual care when appropriate.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Employment Types Available

• Full-time Physical Therapist – In-Home & Senior Living Care (salaried with benefits)
• Part-time Physical Therapist – In-Home & Senior Living Care (salaried with benefits eligibility based on minimum hours requirement)
• PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation)

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
