Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in Springfield, IL to deliver one-on-one geriatric care in patients' homes or Senior Living Communities through the Geriatric House Calls model.

You will evaluate, plan, and provide evidence-based therapy with autonomy over your schedule, including optional telehealth to extend care.

Qualifications

• Licensed Physical Therapist in the state of practice (IL) or eligible to obtain
• DPT or PT degree from accredited program is required
• CPR certification current
• Basic computer and EMR proficiency required

Responsibilities

• Provide one-on-one PT to older adults in homes or Senior Living Communities
• Perform evaluations and develop individualized care plans
• Deliver evidence-based therapy to improve mobility and independence
• Document in FOX's EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Offer telehealth / virtual care when appropriate to extend access

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX's EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Physical Requirements

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including:

• Exerting medium physical effort, including lifting 25-50 lbs occasionally, 10-25 lbs frequently, and up to 10 lbs constantly
• Performing CPR
• Providing moderate to maximum assistance with patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE when required
