Job summary

Fox Rehabilitation in North Port, FL is seeking a licensed Physical Therapist to provide one-on-one geriatric care in private residences or senior living communities. You will deliver therapy that improves mobility, independence, and quality of life, using the Geriatric House Calls model and flexible scheduling.

Clinicians may incorporate telehealth and other virtual care options to extend access, collaborate with a multidisciplinary team, document in the EMR, and build lasting relationships

Qualifications

• Must hold an active Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency required.
• New graduates eligible for FOX’s Emerging Professionals Mentor Program.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Deliver sessions that support mobility, independence, and functional outcomes.
• Document effectively within FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Provide telehealth or virtual care when appropriate to extend access.
