Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric PT in patients' homes and senior living communities in North Port, FL. Clinicians will tailor plans of care, perform evaluations, and help patients improve mobility and independence through our Geriatric House Calls model.

Role offers schedule autonomy, potential telehealth integration, and meaningful outcomes for older adults while collaborating with a multidisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computerized/EMR proficiency and CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence and safety.
• Document effectively within the EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote aging in place and safety.
• Provide telehealth/virtual care when appropriate to extend access.

Responsibilities

• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR systemin accordance withregulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly

• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE whenrequired
