Job summary

Physical Rehabilitation Network, LLC. seeks a licensed Physical Therapist in Albuquerque, NM, to provide high-quality rehabilitation services and patient-centered care.

You will assess, diagnose, and develop individualized treatment plans to improve mobility and function within a supportive multidisciplinary team. You will implement evidence-based therapies, document progress, educate patients and families, and stay current with PT advances to ensure optimal outcomes for diverse patient

Qualifications

• Valid state licensure as a Physical Therapist.
• Doctorate or Master's in Physical Therapy from an accredited program.
• CPR certification and any other relevant certifications as required by state regulations.
• Strong clinical skills with experience in orthopedic, neurological, or geriatric physical therapy (preferred).
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and collaboratively within a team environment.
• Proficient computer skills for documentation and record-keeping purposes.

Responsibilities

• Conduct comprehensive assessments to determine patients' functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
• Implement and administer therapeutic exercises, manual therapy, and other interventions to improve patients' mobility, strength, flexibility, and function based on each individual patient’s need.
• Maintain accurate and organized documentation of patient progress and treatment outcomes.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Educate patients and their families about therapy procedures, home exercises, and injury prevention techniques.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries.
• Stay updated on advancements in physical therapy techniques, technologies, and best practices, integrating them into clinical practice as appropriate.
• Meet the needs of having patients execute Plan of Care and (but not limited) following up with patients if noncompliance occurs.
• Be a team player to utilize nonclinical staff to support needs of the caseload.
• Delegate and use a Physical Therapy Assistant (PTA) when applicable.
• Participate in helping to drive a collaborative culture based on our service vision.
• Adapt to change in patient schedules and daily patterns.Adhere to ethical and professional standards, ensuring compliance with regulatory requirements and company policies.
• Perform comprehensive patient assessments to determine therapy needs and establish treatment goals.
