Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Bachelor's Degree in Physical Therapy (PT)
  • Minimum of Six (6) Months PT Experience
  • PC Experience/Skills

Education, Knowledge, Skills and Abilities Preferred:

  • Doctorate

Licenses and Certifications Required:

  • Physical Therapist License
  • AHA Basic Health Care Life Support HCP Certification
Responsibilities

Responsible for providing professional rehabilitation therapy to patients in their home through evaluation, development, and implementation of individual goal-oriented therapy programs in accordance with organization policies and procedures and external organizations.

  • Complete patient evaluation within the specified time frame and ensure it contains data that reflects a complete, holistic assessment of the client's condition.
  • Develop an individualized treatment program based on evaluation findings, focusing on time-oriented, functional, and measurable short- and long-term goals.
  • Prepare appropriate documentation, such as progress notes, reassessments, and discharge summaries, according to policy and procedures. Ensure that documentation reflects skilled care and monitors the patient's progress towards goals.
  • Ensure documentation is precise, submitted in a timely manner, and supports the need for continued skilled care in the home.
  • Follow the physician's plan of care regarding the frequency and duration of services.
  • Act as a liaison among physician, MCP, and other healthcare team members to ensure effective coordination, appropriate resource referral, and utilization of home health aide services.
  • Supervise the Physician Therapy Assistant (PTA), assess their ability to perform interventions, and ensure co-signatures are completed within regulatory timeframes.
  • Educate patients and/or caregivers with written home programs, and submit copies to the medical record as per policy.
  • Provide comprehensive education to patients and caregivers based on their needs in an understandable manner.
  • Develop rapport and mutual respect with patients to foster an environment conducive to goal achievement. Implement treatment plans with clinical skill and techniques, and meet productivity requirements consistently.
  • Participate in discharge planning throughout the patient's course of care, addressing problem areas appropriately.
  • Complete scheduling requirements accurately and timely, updating and submitting schedules through appropriate systems.
  • Respond promptly to voicemails and messages.
  • Document communication with other healthcare team members throughout the patient's care using secure messaging systems.
  • Ensure discharge summaries accurately reflect the patient's current status, outcomes, and recommendations for further services.
  • Perform other duties or projects as assigned, demonstrating flexibility in responsibilities.
  • Adhere to HMH's organizational competencies and standards of behavior.
  • Lifting, pushing, and pulling a minimum of 50 lbs., and standing for at least 8 hours a day are required.
