This is a Per Diem/PRN position supporting Northbrook and Evanston locations.

Responsibilities

1. Provide physical therapy services based on a physician's plan of care, including assessment and evaluation of therapeutic, rehabilitative, and functional status.
2. Assess muscle strength, mobility, gait, ROM, and rehab potential.
3. Direct physical therapy treatments and instruct patients and families/caregivers on the use of therapeutic appliances.
4. Determine priority needs and report patient reactions to physicians.
5. Initiate therapy programs, train family or caregivers, and participate in case conferences.
6. Identify additional home health service needs and assist in discharge planning.
7. Prepare clinical and progress summaries and provide consultation as needed.

Qualifications

1. Degree in physical therapy from an accredited organization (bachelor's, master's, or doctorate).
2. Pass the National Physical Therapy Examination (NPTE).
3. Licensed to practice in Illinois.
4. At least 1 year of relevant experience; community/home health experience preferred.
5. Excellent communication and organizational skills.
6. Current CPR certification.
7. Reliable transportation.

Compensation & Benefits

Pay range: $34.57 - $43.54 per hour.
