Job Details

Position: Staff Level I Physical Therapist

The Physical Therapist is responsible for evaluating, treating, and planning discharges for all referred patients. They play a key role in helping patients achieve their highest functional level and work collaboratively with healthcare professionals to coordinate care. Job Description

Essential Functions:

  • Conduct therapy evaluations, develop treatment plans, implement interventions, and coordinate discharge planning across various settings such as acute care, skilled facilities, rehab, and outpatient.
  • Participate in student education and contribute to teaching activities suitable for novice clinicians.
  • Design, organize, and deliver treatment programs aimed at patient recovery and rehabilitation.
  • Utilize appropriate tools and techniques to ensure safe therapeutic interventions and training for patients and their families.
  • Document patient progress, including daily treatments, evaluations, progress notes, and discharge summaries.
  • Perform other duties as assigned.
