Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services.

The Physical Therapist is an integral healthcare provider accountable for assisting patients to achieve their highest level of function.

The Therapist will uphold the mission, vision, and values of Jefferson Health.

They work in conjunction with other healthcare professionals to coordinate treatment plans and care.

Essential Functions:

1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload in various settings such as acute care, skilled facilities, acute rehab, and outpatient services.
2. Plan, organize, and deliver treatment programs to help clients/patients recover and rehabilitate.
3. Utilize appropriate tools and techniques to administer safe therapeutic interventions and training for patients and families.
4. Record and document clients'/patients' health progress, including evaluations, daily treatments, progress notes, and discharge summaries.
5. Perform additional duties as assigned.

Job Requirements:

Must possess a Bachelor's, Master's, or Doctoral Degree from an accredited Physical Therapy Program.

Experience of 0-2 years in clinical practice is required; 1 year preferred.

Valid state license in Physical Therapy, along with BLS and CPR certifications are required.
