Job description

Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The therapist is an essential healthcare provider dedicated to helping patients reach their highest level of function. They will uphold the mission, vision, and values of Jefferson Health.

The therapist collaborates with other healthcare professionals to coordinate treatment plans and care.

Essential Functions:
  1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload across acute care, skilled facilities, acute rehab, and outpatient settings as appropriate.
  2. Plan, organize, and deliver treatment programs to assist patients in recovery and rehabilitation.
  3. Utilize appropriate tools and techniques to administer safe therapeutic interventions and training for patients and families.
  4. Record and document patients' health progress, including evaluations, treatments, progress notes, and discharge summaries.
  5. Perform other duties as assigned.
Job Requirements:
  • Bachelor's, Master's, or Doctoral Degree from an accredited Physical Therapy Program.
  • 0-2 years of clinical practice experience required; 1 year preferred.
  • Valid state license in Physical Therapy.
  • BLS and CPR certification via AHA.
