Job summary

Enhance Therapies is seeking a Physical Therapist for a Per Diem role in SNF/Long-Term Care settings through Rehab Alliance. You will evaluate and treat patients, create individualized care plans, and collaborate with a multidisciplinary team to ensure high-quality outcomes.

The position emphasizes flexible scheduling, ongoing professional development, and opportunities for internal transfers within the Enhance Therapies family. New grads welcome with mentorship available.

Qualifications

• Must hold a state Physical Therapist license.
• Degree from an APTA-accredited PT program (BS, MS, or DPT).
• Strong communication and organizational skills; mentorship available for new grads.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Obtain and update all required physician orders.
