Join to apply for the Physical Therapist - Per Diem role at Hackensack Meridian Health

The Physical Therapist evaluates, plans, and implements goal-oriented treatment programs that focus on restoring function and preventing further disability caused by trauma, disease, amputation, congenital defects, surgery, or other illnesses.

Responsibilities include:

  • Participating in committees as requested
  • Delivering high-quality customer service to residents, families, and visitors
  • Maintaining a safe environment and employing correct body mechanics during treatments
  • Developing discharge plans and completing summaries timely
  • Assessing need for DME and educating residents/families on its use
  • Preparing documentation such as progress notes, reassessments, and discharge summaries
  • Gathering evaluation information and providing education to residents and families
  • Handling special projects and rush assignments as needed
  • Developing individualized treatment plans and setting measurable goals
  • Ensuring safety and proper utilization of prosthetic/orthotic devices

Qualifications:

  • Graduate of an APTA-accredited program or equivalent
  • Physical ability to perform required tasks, including lifting and moving heavy objects
  • Excellent communication skills and proficiency with computers

Licenses and Certifications:

  • Physical Therapist License
  • AHA Basic Health Care Life Support HCP Certification
