Job summary

Flagship Rehabilitation, part of Enhance Therapies, is seeking a licensed Physical Therapist for a per diem position in our SNF and long-term care settings. You will evaluate and treat patients with mobility impairments, develop care plans, collaborate with the team, and maintain regulatory documentation.

Qualifications

• Must hold a current Physical Therapist license in the state of practice.
• Excellent communication and organizational skills.
• Experience in long-term care or SNF settings preferred.

• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
