This is a salaried, exempt PT staff position to provide evaluation and individual treatment to assigned residents. Responsibilities include completing all necessary documentation (evaluations, progress notes, discharge summaries, adaptive equipment justification letters, etc.), attending and reporting at all care plan meetings, providing clinical supervision to PT students, and consulting with other disciplines and families as necessary.
Essential Job Functions & Responsibilities

- Provide evaluations and treatments to residents with various medical diagnoses.
- Conduct initial and ongoing evaluations of residents as requested by physicians.
- Complete all documentation, including evaluations, progress notes, annual reviews, and discharge summaries for residents on therapy caseloads.
- Responsible for providing monthly PT statistics timely.
- Coordinate treatment programs with interdisciplinary staff to ensure continuity of care.
- Monitor and maintain adaptive equipment in collaboration with team members.
- Report on residents’ functional levels and progress during interdisciplinary meetings.
- Maintain communication with families and staff regarding resident care.
- Participate in and contribute to in-service training programs.
- Serve as a clinical instructor for PT students at the center.
- Provide therapy sessions in the hydrotherapy pool as scheduled, once CPR certified.
- Upgrade skills through professional seminars and courses.
- Assist in in-service education for staff.
- Establish communication with outside school therapists and disseminate relevant information during care meetings.
- Adhere to standards of customer service, quality improvement, safety, confidentiality, compliance, and policies.
- Maintain daily records, progress notes, and resident schedules, and complete monthly statistics.
- Attend all interdisciplinary meetings and provide updates on residents’ care plans.
- Perform home visit evaluations as needed for discharge or other situations.
- Participate in new hire orientation and provide educational sessions on relevant topics.
- Perform other duties as assigned by supervisors or management.
Qualifications
Education Requirements:
- Master’s degree or doctorate in Physical Therapy
- NYS P.T. License and Registration Certification
- CPR Certification preferred
Experience:
- Minimum of 1 year pediatric PT experience preferred
- Completion of pediatric clinical affiliation
Employee Requirements:
- Fluency in English (reading, writing, speaking, understanding)
- Meet health standards per NYDOH and other agencies
- Meet general health requirements, including a medical and physical exam
- Ability to lift a minimum of 50 pounds
- Ability to handle exposure to blood/body fluids
- Assist in evacuation of residents if necessary

Hourly Rate
$75.00/Hour
