Job summary

Pathways Healthcare in Springfield, VA is seeking a licensed physical therapist for per diem assignments serving Alexandria and surrounding areas. You will assess patients in home health settings, develop rehab plans, and collaborate with physicians, nurses, and families to optimize recovery at home.

Responsibilities include implementing PT plans, educating caregivers, supporting discharge planning, and documenting progress.

Qualifications

• Must have a degree as a physical therapist approved by an accredited organization.
• Minimum two years of relevant PT experience; home health preferred.
• Current BLS/CPR certification, valid driver's license, and reliable vehicle.

Responsibilities

• Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.

Responsibilitie s:

• Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.

Qualifications:

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist (preferred), with priority given to those with community/home health experience is desirable.
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
