Job summary

Pathways Healthcare is seeking a compassionate Physical Therapist to join our home health team in the Londonderry, NH area. You will assess patients in their homes, develop rehab plans, and coordinate with families to maximize functional recovery.

Per diem role serving Salem and surrounding areas. Requires PT degree, BLS CPR, and strong communication skills; prior home health experience preferred. Valid driver's license and reliable vehicle necessary.

Qualifications

• Requires a degree as a physical therapist from an accredited organization.
• Two years of PT experience preferred, especially in home health.
• Strong communication and organizational abilities; BLS CPR certification required.

Responsibilities

• Provide PT services per physician's plan of care and state PT Practice Act.
• Develop and implement individualized therapy programs for patients and train caregivers.
• Instruct on use of therapeutic devices and provide in-service education as needed.

About the Role:

We are seeking a highly qualified and compassionate Physical Therapist to join our dedicated team. As a Physical Therapist, you will have the opportunity to make a significant impact on the lives of our patients by providing exceptional care and support.

Position Details:

• Main Office Location: Londonderry, NH
• Service Area: Salem and surrounding areas.
• Job Type: Per Diem

Responsibilities:

• Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
• Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
• Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
• Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
• Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
• Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.

Qualifications:

• Possess a degree as a physical therapist approved by an accredited organization.
• A minimum of two years of relevant experience as a physical therapist (preferred), with priority given to those with community/home health experience is desirable.
• Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
