Job summary

Mount Carmel Health System in Ohio is seeking a Physical Therapist to evaluate and treat patients referred for physical therapy. The PT collaborates with other caregivers to provide quality care and maintains accurate patient records.

The role requires advanced training, with a Doctorate preferred, Ohio license, and BLS certification. Travel to other sites may be necessary; day shift, full-time.

Qualifications

• Graduate of an accredited physical therapy program; Doctorate degree preferred.

• One year of clinical experience preferred.
• Effective communication and sound judgment in patient evaluation and treatment.

Responsibilities

• Documents PT evaluation results, interventions, progress toward goals, and discharge records.
• Maintains proper billing for PT services.
• Delegates appropriate treatment procedures to the PTA; reviews/co-signs documentation.
• Communicates with health care providers on patient treatment, continuity of care, and discharge planning.

Description

The Physical Therapist (PT) provides evaluation and treatment of patients referred for physical therapy services. The Physical Therapist (PT) communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records. This position requires advanced training, skills, and education to treat both female and male patients.

What You Will Do

• Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.
• Documents accurately PT evaluation results, treatment interventions, patient progress toward goals, and discharge records.
• Maintains proper billing for the services provided.
• Delegates appropriate treatment procedures to the Physical Therapist Assistant/PTA; reviews/co-signs their documentation.
• Communicates with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.

Minimum Qualifications

• Education: Graduate of an accredited physical therapy program; Doctorate degree preferred.
• Licensure / Certification: Current license to practice Physical Therapy in the state of Ohio; Current Basic Life Support (BLS) for Health Care Provider certification required.
• Experience: One year of clinical experience in respective area preferred.
• Effective Communication Skills
• Ability to make decisions and utilize good judgment concerning patient evaluation and treatment.
• Ability to function in a professional capacity with minimal supervision.
• Physically able to perform strenuous activities, such as lifting and transferring patients.
• Ability to travel to other sites as necessary to meet system needs.
