Mille Lacs Health System is seeking a Physical Therapist to perform comprehensive PT examinations and provide treatment aligned with our mission to improve quality of care. Qualifications

• Performs thorough patient examinations prior to initiation of treatment.
• Establishes an individualized plan of care with measurable short-term and long-term goals.
• Provides treatment information to the patient and obtains informed consent before initiating physical therapy.
• Consults with physicians and related disciplines regarding treatment plans, progression, and discharge planning.
• Administers treatment complying with professional standards and facility policies.
• Monitors progress and ensures continuity of quality physical therapy care.
• Provides home care education and discharge planning.

Responsibilities

• Conducts comprehensive PT examinations and treatments focused on patient-centered care.
