Clinton County Medical Center is seeking a licensed Physical Therapist to provide comprehensive PT services. The role involves evaluating patients, developing individualized treatment plans, delivering evidence-based interventions, and documenting progress within a patient-centered care framework at CCMC.

The therapist will collaborate with physicians and the rehabilitation team to promote mobility, reduce pain, and improve quality of life, while adhering to organizational standards and PCMH

Qualifications

• Doctor of Physical Therapy or MSPT degree from an accredited program.
• Active Michigan Physical Therapist License required.
• Strong clinical assessment and treatment planning skills.
• Knowledge of evidence-based physical therapy interventions across populations.

Responsabilités

• Evaluate patients and develop individualized plans of care based on clinical findings.
• Provide physical therapy using evidence-based practices and standards.
• Assess progress and modify plans to reach outcomes.
• Educate patients and caregivers on diagnoses, plans, home programs, and prevention.
• Maintain complete documentation of evaluations, treatments, progress notes, discharge summaries.
• Collaborate with physicians, rehab staff, and other professionals to coordinate care.
• Ensure compliance with policies, standards, payer requirements and regulations.
