Clinton Place is seeking a licensed Physical Therapist to provide comprehensive therapy services within a patient-centered program. You will deliver evaluation, treatment planning, and discharge planning while collaborating with nursing and rehab teams to ensure high-quality outcomes.

The role emphasizes autonomy in the rehabilitation process, ongoing professional development, and adherence to state and federal regulations. An in-house program supports therapists in delivering tailored care.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state Physical Therapist license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Provide physical therapy services, screening, evaluation, and ongoing treatment plans.
• Discharge planning, supervision of assistants and rehab techs, and case management.
• Direct supervision of PTAs and Rehab Techs per state guidelines.
• Participate in care conferences and interdisciplinary team activities.
• Develop and enhance PT programs to support quality outcomes.
