Job summary

Synchrony Rehab at Stone Rehabilitation & Senior Living in Massachusetts seeks a Physical Therapist to provide a full range of PT services, including assessment, treatment planning, and interventions in a collaborative, interdisciplinary setting.

You will assess patients, develop and implement personalized plans, educate patients and families, document progress, and work with the care team to optimize recovery and function while adhering to professional standards.

Qualifications

• Education: Degree in Physical Therapy from an accredited program.
• Experience: 0-1 years.
• Licenses and Certifications: Must have and maintain a current state PT licensure; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehab needs.
• Develop and implement personalized treatment plans with exercises and modalities.
• Educate patients and families on exercises, posture, and body mechanics.
• Maintain accurate documentation of progress and outcomes per policies.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.
