Schedule: Part Time

Shift: Saturday and Sunday, 8-hour Day Shift

The role you’ll contribute:

To evaluate, plan, and deliver individualized, age-appropriate patient care in the field of Physical Therapy.

  • The value you’ll bring to the team:
  • Optimally provides and manages the delivery of patient care, documenting appropriately and accurately on the OASIS/Assessment and visit tools to ensure quality care and proper reimbursement, while complying with all department policies, procedures, practices, and standards of care.
  • Communicates patient’s rehabilitation status with other health care professionals to facilitate interdisciplinary continuity of care and discharge planning.
  • Identifies continuing educational opportunities for professional growth and demonstrates the ability to evaluate suggestions and criticisms objectively, undertaking to improve behavior or seek further guidance as needed.
  • Establishes professional goals in collaboration with supervisor annually and demonstrates compliance with an action plan to address performance deficiencies.
  • Recognizes the influence of personal and cultural values on perceptions of health care and their impact on care delivery. Demonstrates caring behavior and interpersonal connectedness while maintaining professional boundaries.
  • Ensures equitable and beneficial use of resources to meet patient and family needs, and navigates healthcare and community systems to access resources, providing community care across settings.

The expertise and experiences you’ll need to succeed:

  • Graduate of an accredited school in PT
  • Experience preferred
