Job summary

Proforma Health is seeking a Physical Therapist to evaluate and treat patients primarily in Rehab and LTAC hospitals. You will perform initial and discharge assessments, develop treatment plans, and document progress in the EMR.

The role involves coordinating with physicians and the interdisciplinary team, mentoring interns, and ensuring high-quality patient care in a fast-paced clinical setting.

Qualifications

• GA PT license or eligibility, BLS certification, and basic computer literacy are required.
• At least 1 year of experience in Rehab, SNF, LTACH, or Acute care preferred.
• Bachelor’s Degree in Physical Therapy required; Doctoral Degree preferred.

Responsibilities

• Provide therapy education as needed and participate as a member of the interdisciplinary team.
