Job Summary: Evaluates and conducts medically prescribed physical therapy treatment programs to treat movement dysfunction and pain resulting from injury, disease, disability, or other health-related conditions.

Job Highlights:

• Part Time - 20 Hours per week

Responsibilities and Requirements:

• Performs initial and ongoing assessments of patient's condition.
• Establishes, revises, and evaluates a plan of care involving the patient/family.
• Performs therapy interventions utilizing standard physical therapy techniques.
• Provides education to patient/family/staff based on educational needs.
• Participates in discharge planning, provides home instructions, and recommends follow-up care.
• Oversees activities of support personnel.
• Maintains documentation of patient responses and interventions.
• Works safely and maintains alertness.
• Performs other duties as assigned.

Education: Bachelor's, Master's, or Doctorate in Physical Therapy or related field, licensed in the state of practice.

Experience: No experience required.

Physical Requirements: Constant standing, frequent lifting (0-25 lbs), use of hearing, speech, vision, walking, reaching, and occasional bending, stooping, climbing, etc.

Licenses and Certifications: BLS Healthcare Provider certification within 90 days of employment; valid state license in Illinois, Missouri, Oklahoma, or Wisconsin.
