Job summary

Live Well Homecare Agency, LLC is seeking a licensed Physical Therapist to evaluate, plan, and implement therapy programs that improve patient mobility, reduce pain, and prevent deformities.

You will collaborate with physicians, develop treatment plans, document sessions, and provide patient and family education while upholding regulatory standards and delivering compassionate care. The ideal candidate holds a PT degree and active licensure plus BLS.

Qualifications

• Graduate from an accredited physical therapy program (Bachelor, Master, or Doctorate).
• Hold valid licensure as a Physical Therapist and Basic Life Support certification.
• Demonstrate effective communication, customer-focused approach, and team orientation.
• Proficient with basic computer tasks and documentation.

Responsibilities

• Patient Assessment: Develop and update care plans and discharge strategies in collaboration with healthcare professionals.
