The Physical Therapist assesses, plans, organizes, and participates in rehabilitative programs to enhance mobility, relieve pain, increase strength, and prevent or reduce deformity in patients with diseases or injuries. Services are provided throughout the care continuum in a cost-effective, high-quality, and customer-focused manner. Compliance with all relevant regulatory standards is required.

Responsibilities:

1. Patient Assessment
   • Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans tailored to the patient's needs.
   • Develop care plans with input from the patient and/or family.
   • Create realistic discharge plans incorporating patient and/or family goals.
2. Treatment Planning and Care Provision
   • Implement the treatment plan.
   • Choose appropriate treatment activities to help patients achieve their goals.
   • Adjust goals and care plans based on feedback.
   • Provide ongoing education and training to patients and families.
   • Offer discharge instructions, follow-up, and referrals as needed.
   • Serve as a referral source for staff, families, and physicians regarding rehabilitation services and equipment.
3. Documentation
   • Document evaluations and care plans according to departmental guidelines.
   • Record treatment sessions per protocols.
   • Complete and submit all required documentation within guidelines.
