Therapy Partner Solutions in Bowie, MD is seeking a Physical Therapist for our Ortho-Based Outpatient in Home Transition Program. This full-time role focuses on short-term in-home rehabilitation (2-3 visits) before transitioning patients to outpatient care with a specialty in orthopedic cases.

About the Role

We are hiring a Physical Therapist to join our Ortho-Based Outpatient in Home Transition Program in partnership with Luminis Health. This innovative care model focuses on short-term in-home rehab-typically 2-3 visits-prior to transitioning patients to outpatient care. This role offers a unique alternative to traditional home health:

• No SOC (Start of Care) visits

• No case conferences or OASIS documentation

• Orthopedic caseload only

• Focus strictly on evaluation and follow-up visits

You'll see approximately 6-7 patients per day with a goal of 30 visits per week.
