Responsibilities

• Provide ongoing reassessment and modification of treatment plans based on progress.
• Discharge planning and patient/family education, including home programs and self-care recommendations.
• Document evaluations, treatments, and progress notes accurately and timely.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain productivity and knowledge of insurance requirements.

The Physical Therapist evaluates patient's mobility and motor assets and liabilities for functional performance. The Physical Therapist provides goal-directed, activity-oriented treatment to promote optimum independence in functional mobility and daily life tasks. As a member of the health care team, the physical therapist works closely with other rehabilitation disciplines, nursing, physicians, and case managers to provide patient-centered care. Patient/family and community education are essential components of this position.

Key Responsibilities

• Perform complete and thorough evaluations of patients' chief complaints, physical impairments, activities of daily living, and functional limitations
• Develop comprehensive plans of care with appropriate, measurable, and functional goals
• Plan and implement appropriate treatment activities to improve identified physical impairments and functional limitations
• Provide ongoing reassessment and modification of treatment plans in response to patients' progress
• Offer appropriate discharge planning and patient/family teaching, including home programs and recommendations for self-care
• Complete timely and accurate documentation of evaluations, treatments, and progress notes
• Collaborate effectively with other healthcare professionals to ensure comprehensive patient care
• Demonstrate effective time management and meet department productivity standards
• Maintain current knowledge of insurance requirements and regulations
• Participate in professional development activities and mentorship program

Qualifications

• Graduate of an accredited Physical Therapy Program
• Current Massachusetts Physical Therapy license
• Current BCLS (Basic Cardiac Life Support) certification

Pay Range: $38.50 - $53.91 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time.
