DeTar Healthcare System seeks a full-time Physical Therapist for outpatient services in Victoria, TX. The role evaluates, plans, and administers therapeutic procedures for adults and children as prescribed by a referring physician, with regular progress reassessment and precise documentation.

The ideal candidate will establish individualized goals, educate patients and families, and collaborate with an interdisciplinary team while maintaining safety and compliance.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• 0-2 years of experience in a physical therapy setting preferred.
• Licensed PT in the state of employment, BLS certification within 30 days of hire.

Responsibilities

• Conducts comprehensive PT evaluations and develops individualized treatment plans.
• Delivers safe therapy treatments and maintains precise documentation.
• Reassesses progress and adjusts plans as needed.
• Educates patients and families on exercises and post-treatment care.
• Maintains equipment safety and a clean treatment environment.
• Collaborates with referring physicians and the care team to ensure continuity.
• Ensures compliance with facility policies and regulatory requirements.

Job Summary

The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation for children and adults as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.

Essential Functions

• Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans across diverse patient populations.
• Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.
• Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
• Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
• Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
• Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
• Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
• Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
• Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
• Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
• Performs other duties as assigned.
• Maintains regular and reliable attendance.
• Complies with all policies and standards.

Qualifications

• Bachelor's Degree in Physical Therapy required
• 0-2 years of experience in a physical therapy setting required

• Ability to perform comprehensive evaluations and develop effective treatment plans.
• Proficiency in using therapy equipment and technology for documentation and billing.

• Commitment to professional development and adherence to ethical and legal standards.

Licenses and Certifications

• PT - Physical Therapist license in the state of employment required
• BLS - Basic Life Support obtained within 30 days of hire required
