Community Health Systems is seeking a Physical Therapist for a full-time outpatient role in Carlsbad, New Mexico. You will evaluate, plan, and administer therapeutic procedures, establish goals, and document progress to support recovery.

You will collaborate with physicians and an interdisciplinary team, ensure safety and compliance, and maintain records. A PT license in NM and BLS within 30 days are required, with 0–2 years of experience preferred.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• 0–2 years of experience in a physical therapy setting is required.
• Licensed PT in the state of employment is required.
• BLS certification obtained within 30 days of hire.

Responsibilities

• Conducts comprehensive PT evaluations and develops individualized treatment plans.

Job Summary

The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation for children and adults as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.

Essential Functions

• Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans across diverse patient populations.
• Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.
• Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
• Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
• Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
• Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
• Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
• Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
• Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
• Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
• Performs other duties as assigned.
• Maintains regular and reliable attendance.
• Complies with all policies and standards.

Qualifications

• Bachelor's Degree in Physical Therapy required
• 0-2 years of experience in a physical therapy setting required

Licenses and Certifications
