Job summary

Jethro Health System LLC is seeking a compassionate Physical Therapist or Physical Therapist Assistant to provide in-home care across MA. This part-time/per-diem role focuses on maintaining patient independence and improving quality of life in a home-based setting.

Responsibilities include implementing the plan of care, documenting visits, coordinating with the care team, and educating patients and families.

Qualifications

• Associate degree from an accredited Physical Therapy program.
• One year of physical therapy experience preferred.
• CPR certification maintained.
• Possess a valid driver’s license.

Part-Time or Per Diem Physical Therapist (PT/PTA) - Home Health Visits

We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting.

Responsibilities:

• Provides care and services in accordance with the plan of care.

• Assists community members and health field personnel to assess, plan for, and provide needed health and related services.

• Documents care provided in an accurate and timely manner.

• Provides teaching to clients and families.

• Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.

• Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.

• Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company

• Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.

• Attends continuing education programs.

• Other duties assigned by the department manager.

Requirements/Qualifications:

• Associate's degree and graduation from an accredited Physical Therapy program.

• One year of physical therapy experience preferred.

• Possesses and maintains CPR Certification

• Possess valid driver's license

Qualifications:

Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.
