Job summary

Encore Communities is seeking a Physical Therapist to evaluate and plan therapy, and administer prescribed modalities in a patient-centered setting. You will work to restore function, prevent disability, and maximize patient outcomes with support from a multidisciplinary team.

The role requires a PT degree, a valid state license, and strong interpersonal skills for interactions with patients, families, and staff. A flexible, growth-oriented environment with educational assistance is offered.

Qualifications

• Graduate of an approved Physical Therapy program.
• Valid state license required.
• Ability to interact well with patients, families, physicians, supervisor and staff.

Responsibilities

• Evaluate, plan, direct and administer physical therapy modalities as prescribed by a licensed physician.
• Administer treatments and physical agents to restore function and prevent disability after injury or disease.
• Assist patients to reach maximum performance and functioning within their capabilities.
• Participate with other home care personnel in patient care planning.

Responsibilities

• The Physical Therapist is responsible for the evaluation, planning, directing and administering of physical therapy modalities of treatment as prescribed by a licensed physician.
• Administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability.
• Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.
• Participates with all other home care personnel in patient care planning.

Qualifications

• Must be a graduate of an approved educational program in Physical Therapy.
• Possess a valid state license.
• Must have the ability to interact well with patients, family members, physicians, supervisor and staff
