Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and physical therapy needs. Assist in developing the therapy plan of care with the physician.
• Monitor and report the patient's response to treatment and any condition changes to the supervising nurse and/or physician.
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
• Educate the healthcare team, patient, and family/caregivers about the plan of care, limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
