Job description

Job Summary:

Responsible for the physical therapy management of patients under the direction and supervision of the Rehabilitation Services Director.

Union Information:

UNAC/UHCP (United Nurses Association of California/Union of Health Care Professionals) is the collective bargaining representative of Rehab Services positions in Northern California, effective 9/26/2021.

Essential Responsibilities:
  1. Patient Evaluation: Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunction. Establishes treatment goals and plans treatment to achieve these goals.
  2. Discharge Planning and Treatment Implementation: Initiates discharge planning from physical therapy. Implements the treatment program through direct treatment of the patient, delegates portions of the treatment plan to appropriate supportive personnel, and coordinates with other healthcare professionals as needed. Provides advice and education to patients and families. Regularly assesses clinical signs and symptoms to evaluate treatment effectiveness, patient progress, and the need for modifying goals and treatment.
  3. Patient Discharge and Documentation: Discharges patients when maximum benefit has been achieved. Provides accurate, timely documentation of patient management and demonstrates progress toward professional practice criteria.
  4. Additional Notes: Kaiser Permanente conducts routine compensation reviews and reserves the right to reevaluate and modify job descriptions or change employment status from salaried to hourly, with appropriate notice.
