Job summary

Collage Home and Community Rehabilitation Partners seeks a Physical Therapist specialized in neurological conditions to provide patient-centered care in San Marcos, CA, and surrounding areas on a PRN basis.

You will work within interdisciplinary teams delivering in-home therapy, developing integrated treatment plans, and addressing functional goals using evidence-based approaches to produce durable outcomes.

Qualifications

• Bachelor's degree in Physical Therapy and state licensure required.
• Experience in neurologically related disorders preferred.
• Excellent planning, organization, writing, and time management skills.
• Ability to communicate with families, professionals, and co-workers.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.

We are looking for experienced Physical Therapists who want to provide truly patient-centered care in San Marcos, CA and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
