Job summary

HCRC consulting in Fort Wayne, IN is seeking a motivated and compassionate Physical Therapist to join our growing integrative medical practice.

You will perform patient evaluations, develop individualized treatment plans, guide therapeutic exercises, and collaborate with a multidisciplinary team to restore mobility.

Requires graduation from an accredited PT program and an active Indiana license. Base salary $90K–$105K with monthly bonuses, PTO, and comprehensive benefits.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Indiana Physical Therapy license.

Responsibilities

• Perform patient evaluations and physical therapy assessments
• Review medical history, imaging, and physician referrals
• Develop individualized treatment plans based on patient needs and goals
• Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs
• Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications
• Monitor patient progress and perform reassessments
• Maintain accurate and timely EMR documentation
• Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes
