Job summary

A Bachelor’s or Master’s degree in Physical Therapy is required, with preference for applicants who have 2+ years in a SNF rehab environment.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy and active GA license.
• Two years experience in skilled nursing facility rehabilitation preferred.
• Proficient in documentation and inter-disciplinary collaboration.

Responsibilities

• Contributes to a positive work team and problem-solving.
• Supports Rehabilitation Services activities in facility.
• Demonstrates superior customer service and follow-up.
• Provides supervision as defined by Georgia PT board rules.
• Collaborates with Rehabilitation Director and other disciplines for case management.
• Maintains knowledge through seminars and journals.
• Ensures safe working practices and punctual attendance.
• Performs other duties as assigned to support System goals.
