Job summary

Millennium Home Care in Florida seeks a dedicated Physical Therapist to provide in-home skilled PT services under physician orders. You will perform assessments, develop treatment plans, and supervise support staff to help clients regain function and manage pain in their homes.

You will educate families, coordinate care with physicians, and document progress, ensuring adherence to care protocols and discharge planning. Requires Florida PT license and previous home health experience.

Qualifications

• Must hold a current Florida PT license.
• Experience in in-home health is preferred.
• Ability to educate and supervise family and client.

Responsibilities

Millennium Home Care, a leading provider of in-home healthcare services, is looking for a dedicated full-time Physical Therapist to join our team. We offer competitive pay, comprehensive benefits, and a supportive work environment.Provides initial and ongoing comprehensive assessments of the client’s needs including Outcome and Assessment Information Set (OASIS) assessments.Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.Treats clients to relieve pain, to develop or restore function, and, to maintain maximum performance using physical means such as exercise, massage, heat, water, light and electricity.Consults and familiarizes staff regarding the established plan.Educates, supervises, and counsels the family and client in the physical therapy program.Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.Provides the equipment required to carry out the plan.Arranges for out-patient service for procedures that cannot be given at home.Records all evaluation data, treatments and the client's response to therapy intervention on a timely basis.
