Summary:

Constellation Health Services is seeking a caring and compassionate Physical Therapist (PT) to join our team. As a Home Health Physical Therapist, you will be the patient’s partner throughout their journey of restoring motion and ensuring they function at their personal best. Your goal is to maximize the patient’s quality of life and movement potential.

Responsibilities:

• Provide exceptional physical therapy services to patients by assessing and understanding the care plan.
• Evaluate patient's functional status and home environment for hazards or barriers.
• Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plan.
• Maintain and process all documentation in compliance with agency standards using the electronic medical record system.
• Submit all clinical documentation, time records, and other information within the required time frame.

Qualifications:

• Licensed Physical Therapist (PT) in the state of employment.
• New graduates are encouraged to apply; we provide excellent support and guidance to start your career successfully.
• 1 year of clinical and home care experience preferred.
• Graduate degree or higher in physical therapy.
• Oasis experience strongly desired.
• Excellent communication and problem-solving skills.
• Valid driver’s license, reliable transportation, and willingness to cover an assigned territory.
• Proficiency with EMRs, tablets, and computers.
• Ability to work independently with minimal supervision.
• Some rotating weekend work required.
• Willing to work one winter holiday and one summer holiday annually.
