Qualifications

• Licensed in Illinois as a Physical Therapist.
• Excellent communication with patients, families, supervisors, and colleagues.
• Comfortable with computers.
• SNF experience preferred.

Responsibilities

• Perform evaluations and develop plans of care and provide treatment.
• Complete documentation accurately and timely.
• Screen residents who may benefit from therapy.
• Communicate patient status to patients, families, and the care team.
• Supervise and direct therapy assistants as applicable.
• Pursue ongoing education to expand clinical knowledge.
• Provide in-service training to the interdisciplinary team.
• Support and participate in department operations and meetings.
